The shell's stderr was discarded, so a shell that could not start looked like an
immediate disconnect. It now goes to the terminal like other output, followed by
the exit code. An end-to-end test covers a machine with no shell. Builds against
Bamboo v0.1.10, which says why when there is no shell to run.

Bamboo reports where its SSH server listens. When a terminal is opened,
Hammerhead lists the session in the site's config and the agent connects out and
joins that connection to its SSH server; Hammerhead signs in over it. No port
resource or edge is involved. A site whose Bamboo does not report an SSH server
still works through an SSH port. The long poll now returns at once for a session
the agent has not been told about, so a request between two polls is not missed.
The SSH dialog shows whether the SSH server is running, says when no port is
needed, and offers the port only for an ordinary ssh client. Builds against Reef
v0.1.12 and Bamboo v0.1.9.

Hammerhead signs in to the site's Bamboo SSH server through the SSH port an edge
exposes, with a key of its own, and relays the shell to the browser over a
WebSocket (xterm.js, served by Hammerhead). The SSH dialog shows Hammerhead's
public key to add to authorized_keys. Host keys are saved on first use and a
changed one is refused; the terminal opens only from the dashboard's own
origin, is audited, and closes after 30 minutes idle. Errors reach the page as
messages over the socket. An end-to-end test runs a real shell through the
edge, tunnel and Bamboo.
